CMS Proposes CY 2027 Updates to End-Stage Renal Disease Payment System and Quality Incentive Program

Proposed rule CMS-1846-P would revise ESRD PPS rates, AKI dialysis payments, and quality incentive requirements for 2027

The Centers for Medicare & Medicaid Services (CMS) published a proposed rule on June 26, 2026 in the Federal Register that would update and revise the End-Stage Renal Disease (ESRD) Prospective Payment System (PPS) for calendar year 2027, according to the official regulatory filing under Docket No. CMS-1846-P.

The proposal, published by the U.S. Department of Health and Human Services (HHS) through CMS, would also update the payment rate for renal dialysis services furnished by ESRD facilities to individuals with acute kidney injury (AKI). In addition, CMS proposes revisions to the requirements governing the ESRD Quality Incentive Program.

For government contractors and health care service organizations operating in the federal health space, ESRD payment system updates directly affect reimbursement rates for dialysis providers under Medicare, including those operating under federal health service contracts. Changes to the ESRD PPS and Quality Incentive Program metrics typically affect dialysis facility operators, biomedical equipment suppliers, and health information technology vendors tracking Medicare quality performance data.
